(19/08/2026) Thank you for your continued support of the My Health, My School Survey during the 2025/26 academic year. The survey will re-open for pupil access on Monday 7th September 2026 and will remain available until Thursday 22nd July 2027. Schools that already have a My Health, My School Survey account do not need to re-register; existing account details will remain active for the new academic year. Please be aware that parental consent is required for all pupils under the age of 13, including those in Year 8.

Important Update: Parental Consent Now Required for My Health, My School Survey (Year 8 and Below)

Schools must now obtain explicit parental consent for pupils in Year 8 and below before they take part in the survey, in line with GDPR and ISA guidance. Headteachers are asked to confirm their school’s compliance via email to schoolwellbeing@leeds.gov.uk. Support materials, including consent templates, are available on the survey website.
